The City of Richland Center welcomes civic engagement and encourages residents to participate in local governance. The City Clerk-Treasurer's Office serves as the central point for submitting official petitions and a variety of public forms. These tools allow residents, businesses, and visitors to express concerns, engage in city decision-making, request public records, and fulfill reporting obligations.

  1. Concern Report Form

    • Use this form to report general issues or concerns within the city, such as property maintenance, safety, or city services. Submissions are reviewed and forwarded to the appropriate department for action.
  2. Public Comment Form

    • Residents wishing to provide public input at a Common Council or committee meeting must complete this form in advance. Comments may be submitted in writing or presented during the public comment period, subject to meeting rules.
  3. Request for Public Records

    • This form allows you to request access to public records as outlined under Wisconsin’s Open Records Law. Please provide as much detail as possible to help locate the records efficiently.
  4. Room Tax Report Form

    • Required by local ordinance, this form is used by lodging establishments to report and remit quarterly room tax payments. Deadlines and instructions are included on the form.
